The Challenge: Building Multiple Health Programs from a Blank Slate
Our client had a mission: to launch three distinct maternal health initiatives—Care Coordination, Community Health Worker (CHW), and Lactation programs—to serve families across New Mexico. They had the clinical vision but started with a genuine blank slate for each: no staff, no systems, and no established workflows.
The challenge was to design and build a shared operational infrastructure that could support all three programs from the ground up. The goal was to avoid creating three separate, siloed systems and instead establish a unified technological foundation that was efficient, HIPAA-compliant, data-driven, and scalable from the outset. In collaboration with key clinicians who defined the requirements for each program, we set out to build a system that would empower future teams across all three initiatives.
The Solution: A Unified, Data-Driven Ecosystem
We architected a comprehensive, turnkey digital ecosystem before the first program staff were even hired. The solution was designed to enable the team to focus immediately on patient care while providing leadership with powerful insights.
- A Unified Foundation for Coordinated Care: We established a central platform that gives all three programs a single, shared view of patient information. This prevents data silos and ensures that if a family interacts with more than one program, their care is seamless and coordinated. It creates a single source of truth that serves as the operational hub for every team member.
- Automating Administrative Work to Free Up Staff: We eliminated the most time-consuming manual tasks by building custom automation. Processes like patient intake, securing consent signatures, and sending physical mailings are now handled automatically. This drastically reduces administrative overhead, minimizes errors, and most importantly, frees up skilled healthcare professionals to dedicate their time to what matters most: patient care.
- Turning Data into Actionable Insights: We have built a robust data analytics infrastructure to provide program leadership with the insights they need to succeed. The system securely gathers anonymized data, allowing it to measure program effectiveness, deliver concrete results to funders, and identify key health trends. This transforms their daily operations into a strategic asset for continuously improving the quality of care for the community.
The result is a unified, HIPAA-compliant, data-driven infrastructure supporting three distinct programs. New hires for any team step into a mature digital environment, allowing them to be productive immediately and dedicate their time to the community they serve.
